Take Control of Your Health

Summary

The CDSMP is a community workshop designed to give people with chronic conditions (such as arthritis, heart disease, cancer, diabetes, asthma, etc.) and/or their caregivers the knowledge and skills needed to take a more active role in their health care.

Participants learn healthier ways to live, gain confidence and motivation to manage their health and feel more positive about life. The program goal is to help people choose healthy behaviors they would like to follow and design strategies for inco rporating these behaviors into daily life.

Learning Objectives

Increasing Knowledge

  • Participants will learn how to manage their chronic conditions using techniques to control pain, fatigue, emotions, etc.
  • Participants will recognize theearly signs and symptoms of depression.

Increasing Communication Skills

  • Participants will learn effective ways to work with their health care professional
  • Participants will learn how to navigate the health care system.

Positively Changing Behavior

  • Participants will learn how their life-style choices and behaviors impact their health.Participants will learn how to use new tools and feel empowered to manage their healthcare, particularly regarding their use of medications.

Curriculum

  • Lesson One: Overview of self-management and chronic health conditions
  • Lesson Two: Problem solving techniques; benefits of physical activity; preventing falls
  • Lesson Three: Decision making strategies; reducing pain and fatigue; relaxation techniques
  • Lesson Four: Better breathing; good nutrition; communication skills
  • Lesson Five: Medications and making treatment decisions
  • Lesson Six: Working more effectively with health care providers

Wellness Initiative for Senior Education (WISE)

 

Summary

The WISE program is a wellness and prevention program targeting older adults created by the New Jersey Prevention Network. The WISE Program is designed to help older adults celebrate healthy aging, make healthy lifestyle choices and avoid substance abuse.

 

Learning Objectives

1. Increasing knowledge

  • Participants learn how their bodies age and how this process affects their ability to metabolize alcohol and medications.
  • Participants will recognize the early signs and symptoms of depression.

2. Increasing social support

  • Participants will develop an appreciation for cultural and generational diversity, including their own increasing age.
  • Participants will demonstrate increased social guidance and social integration.

3. Positively changing behavior

  • Participants will learn how their lifestyle choices and behaviors impact their health.
  • Participants will learn how to use new tools and feel empowered to manage their health care, particularly regarding their use of medications.

Number of sessions: 6   one hour each

Metro Employee Assistance Service (MEAS)

MEAS provides short term confidential counseling to employees of nearly 20 employers in the Mercer County area. Employees and their family members may access this Employee Assistance Program (EAP) free of charge. Whether it is a minor difficulty or a major obstacle, MEAS licensed professional counselors are available to help. Professional help may make the difference between an early resolution and a long period of struggle.  The majority of clients are self-referrals, but employers may refer an employee when a deterioration in job performance is noted. However, all information, except for compliance issues remain confidential.

Healthy Outlooks for Older Adults: Alternative Approaches to Managing Pain

Summary

This one-hour program will engage older adults and provide them with information regarding appropriate medication use and management and alternative approaches to treating chronic/ acute pain. 

 

Program Benefits

  • Learn about risks of prescription pain medications (opioids) such as Viocodin, Oxycontin, or Codeine
  • Find alternatives to pain management: Acupuncture; Yoga; Physical Therapy; Chiropractic; Massage; Meditation, etc.
  • Gain insights into prescription medicine misuse and safety

We invite you to join the Prevention Coalition of Mercer County - a volunteer organization of citizens dedicated to creating a healthy, safe, community free of substance abuse and related problems. Our mission is to reduce substance abuse, especially among youth, by providing opportunities for coordinated planning, sharing of resources and increased collaboration.
